How to Complete KPAA Applications

  • Please completely fill out and sign the application. This includes days and hours generally available at the premises. Failure to complete the application may result in your license being denied.
  • Please give clear and accurate directions to your premises.
  • If you change your name, or your address during the licensing year, please notify us within 10 days.
  • If you move during the license year, you must reapply for a license for your new premises. Because you will be considered a new facility, the fee may be pro-rated.
  • Remember, licenses are for the premises, not the person. Your license does not allow you to sell animals from an unlicensed premises.
  • If you decide to close, please notify this office with a written statement as soon as possible.
  • If you are not available for an inspection during daylight hours, please state a designated person's name who can be available.

Those with USDA License

If you are USDA licensed, please include a copy of that license with your application. Please note that you must maintain your USDA license during the entire state licensing year. If it expires, your state license is invalid as well.

We will no longer accept USDA "pending" for a license application.

Note: For license year July 1- June 30, you are required to obtain a license to offer, produce, and sell 3 or more litters, including USDA applicants. You are required to obtain a license to maintain more than 19 dogs, cats or both on the premises. Failure to obtain a license may result in administrative action.
