How do I update my address with the Board?
By submitting a copy of a current government issued photo ID which reflects current address or by filling out and submiting the name and address change form to the Board office.
I received a "Notice of Required Remedial Action." What do I do?
Whenever an establishment has four (4) or more violations at an inspection, the establishment is considered to be out of compliance. The Board will then send you a “Notice of Required Remedial Action” to which you must reply.
The second page of the letter is the “Remedial Action Form and Affidavit.” This form lists each of the violations from your last inspection. Look at the questions on your most recent inspection sheet to see what each particular violation is. Then, in the box under “Remedial Action” on the form, write what you have done to correct the violation. You may attach a separate sheet if you need more space.
You must then take the “Remedial Action Form and Affidavit” to be notarized. You will sign the form in front of the Notary and the Notary will then stamp and sign the form. You must then send the form and any additional pages to the Board office. This form should be sent to the Board office within 20 days of the date on the notice.
Please note, you are not being fined at this time. If you receive a “Notice of Required Remedial Action” it simply means that you had a bad inspection and are currently out of compliance. If you correct the violations and do not continue to have bad inspections, you will not be fined. For information about when fines are issued for non-compliance with the health and sanitation regulations, please review the Board Policy.
I lost my license, what do I do?
Complete the duplicate license application, have it notarized and submit to the Board office with the $25 processing fee.
How do I change my establishment name?
You will need to submit an affidavit for change of facility name to the Board office.
I am moving. How do I get verification of my licensure to the state I am moving to?
Complete the licensure and training verification form, and submit it with the $20 processing fee to the Board office.
I am licensed in another state. How do I get my Kansas license?
You must take and pass the Kansas exam and meet the Board’s requirements for licensure. To apply for the examination, complete the out of state examination application and submit the application and the required documentation to the Board office.
If I fail any part of my initial licensure exam is my temporary license still valid?
No. The temporary license is null and void at that moment.
What do I do if I have a name change?
Submit the name and address change form to the Board office with a copy of legal documentation of name change.
How do I acquire a law book?
Complete the law book order form and submit it to the Board office along with your $5 check or money order. You can also access all of the statutes and regulations online.
How do I file a complaint?

Does Kansas have reciprocity with any other states?
No. Every applicant must take and pass the Kansas exam and meet the Board’s requirements for licensure. To apply for the examination, complete the out of state examination application and submit the application and the required documentation to the Board office.

How do I become a licensed Body Art Practitioner?
Kansas law requires that each applicant for Kansas Board of Cosmetology licensure be a high school graduate or have attained the “equivalent thereof.”
You must submit an application provided by the Kansas Board of Cosmetology with the following information:
- A training statement verifying 1,200 hours of training, with the training being provided under the direct supervision of a licensed trainer as outlined in Kansas Administrative Regulation (K.A.R.) 69-15-2;
- Copies of 50 completed release forms for body art services provided by the apprentice during the training. All information on these release forms must be legible for verification of compliance.
- This training verification must include the subject matters and respective instructional hours and outline the reference materials and text books used in training. This information must be completed by the individual who provided the training.
- Verification the trainer was licensed during the applicant’s participation in the training program, as well as the facility being licensed at the time of training;
- Verification that the applicant completed the training program under the direct supervision of the trainer;
- An official transcript of your high school graduation or GED directly from the issuing party;
- A current government issued photo ID (for example, driver’s license) is requested for verification of birth. To fully comply with Kansas law; a clear and legible copy of this document must be submitted; and
- The apprentice must provide verification of completion of eight (8) hours of Board approved training in blood borne pathogens and/or infection control in addition to the required infection control subject matter outlined in K.A.R. 69-15-2.

Once the documents are submitted your application will be reviewed for compliance with Kansas law. At that time, you will be informed of the application status.
You will be required to take the State Board exams. Once compliance is determined you will be contacted regarding examination fees and scheduling of the exam.
Please also be informed that to practice tattoo, body piercing or cosmetic tattoo services in Kansas without a valid Kansas license is a violation of Kansas law and may subject you to legal action. Likewise, a facility that employs an unlicensed individual is in violation of Kansas law and may also be subject to legal action.

Are my antiseptic, germicidal and disinfectant solutions appropriate?

Antiseptics are used exclusively on skin to inhibit bacteria growth, i.e. alcohol or iodine solutions. Historically the iodine solutions, iodine suspended in alcohol, acted as a marking agent, letting the provider know which area was clean. For piercings I have seen the following procedure:
1. Iodine solutions are applied to the area
2. The piercing is marked
3. Piercing is performed
4. Piercing is then cleaned with alcohol before releasing the client.
I was told this was to keep the protocol of clean, mark, pierce and clean. This way, where you are in the process could be easily verified by anyone at a glance and it serves as a reminder for you if you are interrupted.
Germicidal solution is premixed. First, wash to clean the skin of dirt, oils and debris. Use soapy water or a solution of tincture of green soap (green soap). Amazingly, if you read the label, the green soap is never to be used on broken skin, but it has been used forever by tattoo artists.
Disinfectants are used exclusively on surfaces to remove or neutralize bacteria, fungi and viruses. There are several brands on the market with varying capabilities. Board regulations require that you use a disinfectant that has demonstrated tuberculocidal activity. For cleaning electrical instruments, Board regulations require that you use an EPA-registered disinfectant with demonstrated bactericidal, fungicidal, and virucidal activity.
Remember, EVERY PROCEDURE IS A BLOOD SPILL, we just try to control it. Whether it is a droplet or a puddle, it falls under 29 CFR 1910.1030. If you don't use barriers it has to be disinfected. Board regulations require that you use an EPA-registered disinfectant with demonstrated bactericidal, fungicidal, and virucidal activity.

Hearings
How do I request a hearing?
Our office requires a written request which can be made via mail, fax, or email. Send your request to the Executive Director and sign it. Include your case number, make your request, and include the reason(s) you are making the request. Visit our Contact Us page for information on where to send your request.
When will my hearing take place?
Hearings take place at the Kansas Board of Cosmetology. Felony review hearings are held the second Monday of each month. All other hearings are scheduled at the discretion of the Office of Administrative Hearings.
How long do I have to request a hearing?
You have 15 days from the date printed on the Certificate of Service to request a hearing.

Fines
How do I pay my fine?
You can submit a check or money order to our office in the regular mail. Credit card payments may be accepted via mail, fax, or email. You must include your name, case number, credit card number, expiration date, amount of payment, and signature.
What if I can’t pay the whole fine right now?
We are happy to work with you! Write a letter to our office requesting a payment plan. Payments have to be made on a monthly basis. Let us know how much you want to pay by a specific date each month and we’ll send you a Payment Agreement and Schedule to sign and return with your first installment. Do I have to pay my fine?
Yes! Outstanding fines cause a hold to be placed on your facility license and/or your personal license. KBOC will not process any request or renewals on these licenses until outstanding fines are paid in full. Additionally, failure to make timely payment may result in your case being turned over to the Kansas Debt Set-Off program and revocation of your license.

Felony Reporting Mistakes
I received a letter asking for information about a felony conviction and I have never been convicted of a felony. What should I do?
Send a signed letter to the Board stating that you made a mistake on your application and that you have never been convicted of a felony. You may send this by mail, email, or fax. Visit our Contact Us page for information on where to send your letter.
What if I submitted my apprentice license application with a mistake?
It is your responsibility to review and revise any document that you sign. See K.S.A. 65-1908(a) (4) and (5). If the "yes" box was accidently checked on your application, please send a signed letter to the office stating your mistake and stating that you have not been convicted of a felony.
