Change of Address

In order to maintain the most accurate information in your licensing file, the Board requests that all licensees notify this office within 10 days of an address change. Notification of change of address shall be submitted in writing with an attached copy of the licensees current government issued photo identification which includes the updated address. Address changes shall be submitted in writing and can be sent via fax to 785/296-3002, e-mailed to kboc@kboc.state.ks.us or sent to the Board office listed below. Please understand that notifying the post office does not automatically notify us of an address change.
