How do I update my address with the Board?
By submitting a copy of a current government issued photo ID which reflects current address or by filling out and submiting the name and address change form to the Board office.
I received a "Notice of Required Remedial Action." What do I do?
Whenever an establishment has four (4) or more violations at an inspection, the establishment is considered to be out of compliance. The Board will then send you a “Notice of Required Remedial Action” to which you must reply.
The second page of the letter is the “Remedial Action Form and Affidavit.” This form lists each of the violations from your last inspection. Look at the questions on your most recent inspection sheet to see what each particular violation is. Then, in the box under “Remedial Action” on the form, write what you have done to correct the violation. You may attach a separate sheet if you need more space.
You must then take the “Remedial Action Form and Affidavit” to be notarized. You will sign the form in front of the Notary and the Notary will then stamp and sign the form. You must then send the form and any additional pages to the Board office. This form should be sent to the Board office within 20 days of the date on the notice.
Please note, you are not being fined at this time. If you receive a “Notice of Required Remedial Action” it simply means that you had a bad inspection and are currently out of compliance. If you correct the violations and do not continue to have bad inspections, you will not be fined. I lost my license, what do I do?
Complete the duplicate license application, have it notarized and submit to the Board office with the $25 processing fee.
How do I change my salon name?
You will need to submit an affidavit for change of facility name to the Board office.
I am moving. How do I get verification of my licensure to the state I am moving to?
Complete the licensure and training verification form, and submit it to the Board office with the $20 processing fee.
I am licensed in another state. How do I get my Kansas license?
Complete the out of state application and meet the Board’s requirements for licensure.
If I fail any part of my initial licensure exam is my temporary license still valid?
No. The temporary license is null and void at that moment.
What do I do if I have a name change?
Submit to the Board office the name and address change form with a copy of legal documentation of name change.
How long will it take to receive my license?
Please allow ten business days for the processing of your licensure application once it is received in the Board office. Although this is an office standard, it is not a guarantee. At that time you will be notified in writing of the status of your application. If the application is incomplete, the notification will explain what will need to be done to complete the missing portion of application.
Once the application is complete you will be notified in writing within five business day what steps you will need to take to complete your examination(s). If you must take the board written and practical exams, the duration of your wait to test will depend on when the Board exams are next to be administered. You will be notified in writing of your test date by the testing company.
How do I acquire a law book?
Complete the law book order form and submit it to the Board office along with your $5 check or money order. You can also access all of the statutes and regulations online.
How do I file a complaint?
Complete the complaint form, and submit it to the Board office by mail, email, or fax. Visit our Filing Complaints page for information on where to send your form.
Does Kansas have reciprocity with any other states?
No. Every applicant must complete the out of state application and meet the Board’s requirements for licensure.

Hearings
How do I request a hearing?
Our office requires a written request which can be made via mail, fax, or email. Send your request to the Executive Director and sign it. Include your case number, make your request, and include the reason(s) you are making the request. Visit our Contact Us page for information on where to send your request.
When will my hearing take place?
Hearings take place at the Kansas Board of Cosmetology. Felony review hearings are held the second Monday of each month. All other hearings are scheduled at the discretion of the Office of Administrative Hearings.
How long do I have to request a hearing?
You have 15 days from the date printed on the Certificate of Service to request a hearing.

Fines
How do I pay my fine?
You can submit a check or money order to our office in the regular mail. Credit card payments may be accepted via mail, fax, or email. You must include your name, case number, credit card number, expiration date, amount of payment, and signature.
What if I can’t pay the whole fine right now?
We are happy to work with you! Write a letter to our office requesting a payment plan. Payments have to be made on a monthly basis. Let us know how much you want to pay by a specific date each month and we’ll send you a Payment Agreement and Schedule to sign and return with your first installment. Do I have to pay my fine?
Yes! Outstanding fines cause a hold to be placed on your facility license and/or your personal license. KBOC will not process any request or renewals on these licenses until outstanding fines are paid in full. Additionally, failure to make timely payment may result in your case being turned over to the Kansas Debt Set-Off program and revocation of your license.

Felony Reporting Mistakes
I received a letter asking for information about a felony conviction and I have never been convicted of a felony. What should I do?
Send a signed letter to the Board stating that you made a mistake on your application and that you have never been convicted of a felony. You may send this by mail, email, or fax. Visit our Contact Us page for information on where to send your letter.
What if my school submitted my apprentice license application with a mistake?
It is your responsibility to review and revise any document that you sign. See K.S.A. 65-1908(a) (4) and (5). If the "yes" box was accidently checked on your application, please send a signed letter to the office stating your mistake and stating that you have not been convicted of a felony.
