69-3-1 Application procedure.
An applicant for a license to conduct a school of cosmetology, esthetics, electrology
or manicuring shall submit the following to the board at least 60 days before
the proposed date of operation:
- A written application upon a form approved and furnished by the board;
- a descriptive floor plan to scale which demonstrates compliance with K.A.R.
69-3-3;
- a curriculum which demonstrates compliance with K.A.R. 69-3-8;
- a daily class schedule for full-time and part-time students;
- an inventory of all instructional equipment to be provided and used in
the operation of the school; and
- a copy of the written enrollment agreement between the school and the student
including the refund policy and the rules and regulations of the school.
