69-5-14 Application procedure.

An applicant for a license to instruct electrology in a shop shall submit the following to the board at least 10 days before beginning instruction and training:

1. A written application upon a form approved and furnished by the board;
2. a curriculum which demonstrates compliance with K.A.R. 69-3-8(a)(4);
3. a daily class schedule for a full-time or a part-time student; and
4. an inventory of all instructional equipment to be provided and used in the instruction and training.
