Number: 66-6-9

Title: Cancellation of license or certificate of authorization.

(a) If any individual or corporation fails to pay the renewal fee within 60 days of the expiration date, the individual's license or the corporation's authorization shall be cancelled.

(b) A new license may be obtained in the manner prescribed for new applicants, unless the original license is
reinstated by the board for good cause shown and upon payment of the reinstatement fee.
